Trang Province is a province in the southern part of Thailand. Its economy is characterized by services, agriculture, livestock, tourism, industry, and commerce. It has a great deal of ethnic and cultural diversity. It is a multicultural community with Thai, Chinese, Muslim, and Christian races. There are temples, shrines, and mosques. Trang is a tourist city because Trang has abundant natural attractions such as the sea, mountains, waterfalls, and caves. Trang's tourist attractions are numerous and attractive. The physical geography is a combination of plateaus and hills. The green mountains are the source of streams and rivers. Trang has more than 40 islands, such as Libong Island, which is known as a habitat for dugongs. Koh Kradan, with its crystal clear waters, was awarded the island with the most beautiful beach in the world in 2023.

According to the population statistics of Trang Municipality as of July 2024, there were 54,344 people in the Trang Municipality area, with the largest population groups divided into 3 ranks as follows: First, the adult group at 52.63 percent, second, the elderly group at 22.23 percent, and third, the teenager group at 18.09 percent. Currently, Trang Municipality has entered a "complete aging society" and it is expected that in 2029, Trang Municipality will enter a super-aged society with an increase in the elderly population of 28.44 percent.

Rationale:
Advance care planning is a process that respects the individuality of the individual by involving the individual with health care personnel in making decisions about their own care. People who are not sick or patients can plan advance care to decide on the way to take care of their own health in the future as desired. In caring for patients with severe chronic diseases that cannot be cured, including caring for the elderly, advance care planning is very important because if patients and their relatives jointly plan advance care with health care personnel, health care personnel will know the goals and needs of the patient and family's care.

3. To practice calmness and good communication.
